Clinical laboratories and biotech companies with new medical laboratory tests are struggling to win coverage by Medicare and private payers. How big is this problem? There are currently tens of thousands of molecular diagnostic assays and genetic tests offered for clinical use.
Any lab company seeking to obtain an appropriate Current Procedural Terminology (CPT) code, favorable coverage guidelines, and adequate reimbursement from health insurers for its new lab test faces three big challenges, and they are related. First, payers are simply overwhelmed with requests to review new genetic tests. The flood of new test submissions exceeds the capability of payers to respond.

State governor declares ‘state of emergency’ and asks central government for aid to get healthcare facilities reopened in city that will host 2016 Summer Olympics
As U.S. healthcare officials look to contain the spiraling cost of care in this country, government leaders in Brazil are facing an unprecedented healthcare crisis that has caused cash-strapped public hospitals in Rio de Janeiro to close their doors or limit services due to lack of funds.

Clinical laboratory executives and pathologists should take note of this unfolding story. It may be without precedent that public hospitals in the capital city of one of the world’s faster-growing economies have had to cease medical services because there is no money to pay physicians and staff and buy the necessary medical supplies.
Luiz Fernando Pezão, the current State Governor of Rio de Janeiro, on Dec. 23, 2015, declared a “state of emergency” after as many as 15 outpatient clinics closed and hospitals began limiting their admissions to those with life-threatening conditions. (more…)

Pathologists and clinical lab managers will not be surprised to learn that Epic leads the competitive electronic health record system market, as ranked by SK&A
No one will be surprised that, in one company’s rankings of the top electronic health record (EHR) systems for 2015, the number one position is held by Epic Systems Corporation. More broadly, about half the market share of EHR systems is concentrated among just five EHR vendors.

Some health IT experts criticize the Government Accountability Office report for ‘incomplete research’ and failure to focus on ‘person-centered interoperability’
Several years after paying billions of incentive dollars to thousands of hospitals and physicians to encourage adoption of electronic health records (EHRs), federal officials remain frustrated at the lack of interoperability among the competing EHR systems. This is a problem recognized by clinical laboratories that must create and maintain interfaces between their laboratory information systems (LISs) and the EHRs of their client physicians.
Frustration over this situation motivated Lamar Alexander (R-TN), Senate Health, Education, Labor and Pensions Committee chairman, along with four other U.S. Senate Committee Chairman, to request that the General Accountability Office (GAO) study the problem and report its findings. The GAO released its report last September in a publication: “Nonfederal Efforts to Help Achieve Health Information Interoperability.”
The GAO’s investigators outlined five barriers to EHR interoperability. They also suggested that meaningful use (MU) requirements present a roadblock to information sharing. (more…)

Recently-announced partnerships want to use big data to improve patient outcomes and lower costs; clinical laboratory test data will have a major role in these efforts
In the race to use healthcare big data to improve patient outcomes, several companies are using acquisitions and joint ventures to beef up and gain access to bigger pools of data. Pathologists and clinical laboratory managers have an interest in this trend, because medical laboratory test data will be a large proportion of the information that resides in these huge healthcare databases.
For health systems that want to be players in the healthcare big data market, one strategy is to do a risk-sharing venture with third-party care-management companies. This allows the health systems to leverage their extensive amounts of patient data while benefiting from the expertise of their venture partners. (more…)
